Submitting the form does not create a confirmed appointment or establish a clinician-patient relationship. The office will contact you regarding availability.
Medical emergencies
This website is not monitored for emergencies. Call 911 or go to the nearest emergency department for a medical emergency.
Third-party services
The website may link to services operated by other organizations, such as FollowMyHealth, mapping services, and the appointment-form delivery provider. Those services have their own privacy practices.

The current launch package does not enable advertising trackers. Any future analytics service should be reviewed and disclosed before activation.
